Visa application form (signed)
The form you filled in online, with its unique code, printed and signed.
Travel medical insurance (Schengen: at least EUR 30,000 cover)
At least EUR 30,000, valid in every Schengen country for the whole stay, covering hospital and emergency care and repatriation. Your sponsor in the Netherlands may buy it for you.
Return ticket reservation and accommodation booking
A return ticket reservation and a hotel reservation or rental agreement for the whole stay (staying with a host: see the invitation item).
Computerised National Identity Card (CNIC)
A copy of your CNIC, as the Pakistan checklist the Ministry links asks.
Family Registration Certificate (NADRA)
NADRA Family Registration Certificate in English, as the Pakistan checklist asks.
Bank statement / proof of funds
Bank statements: the Pakistan checklist asks for the last six months, signed and stamped by the bank (the general Dutch checklist says three months).
FBR income tax return / wealth statement
Your NTN certificate (or proof of exemption) and the FBR acknowledgements of your tax returns for the last two years, as the Pakistan checklist asks.
Needed in some situations
Invitation letter from host or business
Family or friends: the IND's proof of sponsorship and/or private accommodation form, signed by your host and the signature legalised at their municipality in the Netherlands, one form for each visitor. Business or events: an invitation with your details, the dates and the reason.
Salary slips
Your last three salary slips.
Employer reference or leave letter (NOC)
A recent statement from your employer with your job title, contract length and salary, or a certificate of employment. The Pakistan checklist also asks civil servants for a no-objection certificate.
Letter from the treating hospital or clinic abroad
A medical certificate, a declaration from the hospital or doctor in the Netherlands with the treatment and its costs, and proof that you paid in advance.
Polio vaccination certificate (NIMS/NADRA)
Pakistan exit requirement, not a visa document: when you leave Pakistan you must show a NIMS/NADRA vaccination certificate including polio (the old paper card is no longer accepted, and since 14 September 2026 airports check a NADRA-integrated vaccination record). Verify current rules.
