Valid for at least one more year, with two blank pages, issued within the last 10 years. Original and a copy of the data page.
Visa application form (signed)
National visa application form, every section completed and signed; a parent or guardian signs for a minor. If you and your family member both live abroad, also form EX24 for the residence authorisation, with your address in Spain.
Passport-size photograph (destination specification)
One recent colour passport-size photo, light background, face fully visible.
Spanish family member's passport or DNI (copy)
Full copy of your Spanish family member's valid passport or DNI.
Medical certificate (public health)
Medical certificate that you have none of the diseases with serious public health effects under the International Health Regulations 2005.
Computerised National Identity Card (CNIC)
Proof that you live in the Embassy's district, for example your CNIC (original and a copy).

Registration in a public register of an EU or EEA state or Switzerland, or proof of living together for at least 12 months (or birth certificates of a child together).
Birth certificate (NADRA CRC or union council)
Child: your birth certificate (under 18 and a child of only one of them: the other parent's notarised consent). Parent: the birth certificate of your Spanish family member or their spouse or partner. Parent of a Spanish child: the child's birth certificate.
Proof of dependency on a family member
Proof that you depend on your Spanish family member (and, for an adult child, that you are unmarried), or of the disability or humanitarian reason. A relative who will care for them: proof of the family link (up to second degree) and the decision recognising their dependency.
Police character certificate
Police certificate from the countries you lived in during the last five years.
Polio vaccination certificate (NIMS/NADRA)
Pakistan exit requirement, not a visa document: when you leave Pakistan you must show a NIMS/NADRA vaccination certificate including polio (the old paper card is no longer accepted, and since 14 September 2026 airports check a NADRA-integrated vaccination record). Verify current rules.

If your Spanish family member lives in Spain, they apply there first for your residence authorisation; you then apply for the visa within one month of its approval. If you both live abroad, you apply for the authorisation together with the visa at the Embassy, on form EX24 with your address in Spain.
