Documents you need

From our draft document list for this visa. Your own list depends on your situation, so check the official source.

Usually needed

  • Passport

Original and a copy of the data page.

  • Visa application form (signed)

Schengen application form, every section completed and signed; a parent signs for a child.

  • Passport-size photograph (destination specification)

One recent colour passport-size photo, light background.

  • EU, EEA or Swiss family member's passport or identity card (copy)

Copy of the passport or identity card of your EU, EEA or Swiss family member.

  • Return ticket reservation and accommodation booking

Proof that you travel with or join your family member: flight tickets, a notarial deed or a sworn statement.

Needed in some situations

  • Nikah Nama / Marriage Registration Certificate

Marriage certificate, apostilled and translated by a Spanish sworn translator. The Embassy may also ask for the marriage record of the EU country (not older than 6 months).

  • Birth certificate (NADRA CRC or union council)

Child: your birth certificate. Parent: your EU family member's (or their spouse's) birth certificate. If your link is through the EU citizen's spouse, add that marriage certificate too. Apostilled and translated by a Spanish sworn translator.

  • Proof of partnership (registration, or living together)

Registration of the partnership in an EU or EEA state or Switzerland, or proof of living together for at least a year or of a child together.

  • Proof of dependency on a family member

Proof that you depend on your family member (for example regular money transfers) or live in their household, or of the health reason.

  • Polio vaccination certificate (NIMS/NADRA)

Pakistan exit requirement, not a visa document: when you leave Pakistan you must show a NIMS/NADRA vaccination certificate including polio (the old paper card is no longer accepted, and since 14 September 2026 airports check a NADRA-integrated vaccination record). Verify current rules.
